ISEF FORMS

...are due on Friday. :)

Please visit https://sites.google.com/a/pshs.edu.ph/diliman-str/home/forms
to get copies of the ISEF forms, print them out and have them signed by all
the relevant people. Put them in a brown envelope, and give 'em to me by
Friday, 02 September 2011.

For all groups, you must all have forms 1A, 1B, 1, and 2 prepared and
signed. At the very least, fill in your name(s), and sign all relevant
entries for you (as student), have your parents sign the relevant entries
(Form 1B), and have a "qualified scientist" (someone with a master's degree
relevant to the thing you're working on) sign Form 2
* 1A: One form per group. Leave adult sponsor blank
* 1B: One form per member. Have a parent or legal guardian sign relevant
area
* 1: One form per group. Just fill in your names and title
* 2: One form per group. Before hassling your contact to sign, make sure
that they have a master's degree or PhD in the field they're working on.
This is relevant for groups working off-campus laboratory sites. DO NOT
HASSLE THEM INTO SIGNING THE PAPER. If you've contacted them before, ask
politely if they can sign the form (because it's a requirement). If not, be
prepared to give a short explanation of your project, and leave a copy of
your revised abstract with them (if necessary) before signing.
* 3: One form per group. Fill up only if working with hazardous chemicals
(check MSDS), biological agents (check BSL), or equipment. Identify and list
risks involved.
* 4: One form per group. Fill up only if working on people. (No groups are
doing that in Graviton, so, yay)
* 5A and 5B: One form per group. Fill up with working on vertebrates other
than people. (Mouse groups, I'm looking for this)
* 6A: One form per group. Fill up if using BSL2+, other biohazardous stuff,
or samples from people. (Again, I don't remember anyone in the class with
this)
* 6B: One form per group. Fill up if also filling up 6A.
